3. Function and Use
It is a powerful macrolide immunosuppressant. At the molecular level, the effect of this product is mediated by the protein FKBP12 which binds to it in the cytoplasm. FKBP12 enables the product to enter the cell and form a complex that competes specifically with calmodulin and inhibits calmodulin, which mediates a calcium-dependent inhibitory signaling system in T cells, thereby preventing a series of lymphokine gene transcriptions. In vivo and in vitro experiments have shown that tacrolimus is a potent immunosuppressant, which can inhibit the formation of cytotoxic lymphocyte. Transplantation rejection is mainly caused by the latter.
The drug inhibits T cell activation and TH cell-dependent B cell proliferation, as well as lymphokine production such as interleukin-2, interleukin-3 and interferon-beta, and the expression of interleukin-2 receptor. Tacrolimus can inhibit the rejection of skin, heart, kidney and liver transplantation and prolong the survival time of allografts. It has been proved in rodents, dogs, primates and humans. Since its launch, it has been widely used in the treatment of acute and chronic rejection of liver, kidney and bone marrow transplantation.
